ACTIVITY ENROLLMENT FORM
This form must be completed and turned into the YES office along with appropriate payments in order for enrollment for events/activities to be completed. Click on the above link to download a copy of the form to print out.

WAIVER FORM
This form must be completed by all coaches of all team sports.

DANCE ENROLLMENT
This form must be filled out for anyone wishing to enroll in any Divine Expressions Dance Studio class.

KIDS ONLINE ENROLLMENT
Any easy and convienent online registration is available to use. Click the above link to register your child for YES events/activities. After completing the online form you will still be required to print and submit (with appropriate payment) the above Activity Enrollment Form.

ONLINE VOLUNTEER ENROLLMENT
YES has many opportunities for individuals to volunteer. If you would be interested in voluteering, please follow the link to our online enrollment page. We will contact you in an appropriate time as soon as we recieve your request.
